This document collection letter template is for anyone authorising another person to collect a document on their behalf. It includes spaces for your full name, address, phone, email, date, the organisation or office address, your date of birth, ID number, the authorised person’s full name and ID number, the document name, application or reference number, and the authorisation start and end dates. It also includes a note that copies of both IDs are attached, plus contact details for confirmation.
